Food Allergy Forms and Resources
Please notify the Willow Ave Health Office of any changes to your student's health status or new medication, food, or insect allergies.
Food allergy forms and doctors orders are good for ONE school year and must be resigned and resubmitted each year.
Medications should be brought into school by a parent, guardian, or trusted adult. For student safety, medication may not be brought in by your student or given to the bus driver.
