General Information

Please note, all student injuries must be reported to the nurse’s office. Students cannot come to school with crutches, wheelchairs, etc…unless they have provided the school nurse with a note from a doctor. Related files and documents can be found at the bottom of this page.
Medical Attention

In order to best meet your child’s health and educational needs in school please inform the school nurse of any health related issues or current medication. Please contact the school nurse via phone 845-534-8009 ext. 5010, or in person. Please provide the Cornwall Central High School with current phone numbers and emergency numbers. It is very important that the school be notified of any change in emergency numbers.
Absence Due to Illness

Please call the attendance line (Ext 5009), if your child will be absent from school. Please notify the Health Office if a serious medical condition has occurred.
Medication Administration

It may become necessary for you child to receive medication during the school day. The following is the procedure for medication administration in the school setting.
A written order from your Health Care Provider is required for ALL medication, both prescription and over-the-counter (Ex. Tylenol, Advil). The medication must be brought to school in the original, labeled container. Loose medication that is wrapped up will not be administered. Before sending in any medication (i.e. nebulizer ampules, pills, Epi-pens, inhalers, ointments, eye drops etc.) please check the expiration date and make sure that the medication will not expire during the school year. We are unable to administer expired medication. Self-Administration of medication is prohibited in school with the exception of inhalers as treatment for asthma and Epi-pens for emergency situations.
Consent forms must be filled out by the Health Care Provider and the parent. These forms must be kept on file in the Cornwall Central High School Health Office. The consent form for medication administration in school is current for only ONE SCHOOL YEAR. Parents must provide the health office with a current doctor’s order and parent permission form each September. Parents must pick up all medication or make arrangements to pick up medication by the last day of school. All remaining medication will be discarded.

Mandated Health Screenings

NO SCREENING FOR 2020-2021 school year
The district's School Health Services program supports your student's academic success by promoting health in the school setting. One way that we provide care for your student is by performing the health screenings as mandated by the State of New York
During this school year, the following screenings will be required or completed at school:
- Scoliosis – Grade 9 Males only
- Vision – Grade 11
- Distance & Near Hearing – Grade 11
Immunizations

All students must provide proof-of-immunization prior to entering or attending school. Immunizations must be current. New entrants will be given 30 days to provide a record of immunizations. If this is not adhered to, students will be considered out-of-compliance and excluded from school until documentation from a Health Care Provider is on file in the Cornwall Central High School Health Office. If you need assistance with obtaining immunizations, please call Orange County Health Dept. at 360-6587.
Immunization records can be faxed to the Cornwall Central High School Health Office at 845-314-9203.
Physical Exams

It is mandated by New York State Law that all 9th - 12th grader students trying out for a school sport, and new entrants into the district, are required to have a physical. The physical must be kept on file in the Cornwall Central High School Health Office. Physicals expire after one year. A school physical will be given by the Nurse Practitioner for the school district if a private physician’s physical is not provided to the Health Office.
Allergy to Nuts: We have one designated table in the Cafeteria solely for children who have an allergy to nuts. Please notify the school nurse if you child needs preferred seating in the cafeteria.

Athletics
Dear Parent/Guardian,
Clearance from quarantine may be obtained by the student's medical care provider or our district medical staff (school nurse). If a student has been quarantined, completes their required quarantine period, and has no symptoms, they do not require a note from a medical provider or a negative test to return to school.
Prior to returning to the school or school-related activities, a parent/legal guardian must fill out the form listed below (follow the link below) and submit it to be reviewed by our school nurses for clearance. Please read through the entire form and answer all of the questions. Each student needs their own form to be completed. Once you successfully submit the Google Form, titled Covid-19 Quarantine return to school, you will receive a confirmation at the end that states “cleared to return, quarantine complete and asymptomatic”. No further action is necessary.
This form may be filled out once your child has completed the quarantine period as indicated by NYSDOH, OCDOH, or shared by school officials. This form must be submitted prior to your child re-entering school for any reason, including athletics and extracurricular activities.
If, upon review, our nurses have any questions they will reach out directly to the parent/guardian. Once you have received your confirmation at the end of the form, there is no need to follow up.
Please note that due to recent COVID exposure, the earliest this form should be submitted for clearance is 1 to 2 days prior to returning to the school building.
This form cannot be used to clear students who have tested positive for COVID-19, those students will still need to provide a doctor's note to our health office prior to returning to the building.
